Preheat the oven to 350°F (175°C).
In a large mixing bowl, combine the refried beans, softened cream cheese, sour cream, and taco seasoning. Mix until smooth.
Spread the mixture evenly into a 9x13 inch baking dish.
Sprinkle the shredded cheese over the bean mixture in an even layer.
Bake for 20 to 25 minutes or until the cheese is melted and the dip is bubbling around the edges.
Remove from the oven and garnish with sliced jalapeños and green onions.
Serve warm with tortilla chips.
